How wrong would I be for this?

Location: Indiana ​ Our current lease is up July 10th, we are planning to move by beginning of July. ​ When we moved in here our landlord was notified by her last tenant that there was mold. She also was aware of the gutters needing done, windows being replaced, the soffit and fascia boards are rotted and need replaced. The carpet in the main bedroom was trashed and needed removed (which we helped her with and removed for her and she said she would get it sanded down and stained) She assured us that she would get these fixed as soon as possible. ​ We also during our time here informed her that a wall in the dining room has water damage, probably growing mold in it and could easily push a hole in it with little force because of the damage, we've also informed her that part if the roof is sinking in and leaking water. ​ Recently we also notified her that the ceiling in the bathroom was sustaining water damage, the paint is peeling and the ceiling itself is crumbling. ​ Last year 8 out of 11 windows were replaced, the last 2 were replaced in December but they never got sealed, which allowed for snow to come in around the windows during the winter. Its almost July and they still haven't been sealed, the other 3 haven't been replaced. The soffit and fascia are still rotted, birds are getting into the attic from that. Still no gutters on the house. Her dad did the electrical in the home and its all rigged up and unsafe (fiance is a lineman, been an electrician as well and took a look at the electrical) The bathroom still has mold everywhere and the ceiling is crumbling (which she blew off and said the paint peeled when she lived her as well, even tho I told her that its not just paint, it's the actual ceiling as well). The bedroom floor still hasn't been sanded or stained. The storm door was broken and she never replaced it, we had to take it off and replace the weather stripping around the door because snow was coming in the sides and she wasn't doing anything. She hasn't done anything about the roof sinking in and when we contacted her a few weeks ago about the ceiling leaking water she didn't care, no one has come to fix it or look at it. She's done nothing about the water damage on the wall in the dining room. ​ Her "handyman" had came twice in the last 3 months and I didn't get a notice either time. Her boyfriend verbally attacked me because I told her that a 24hr notice was required before he came out to make repairs. The handyman didn't even do anything those 2 times that he came out. She refuses to hire someone else to do the work because this guy is cheap. So far we've paid her $11,400 in the last year, counting security deposit. Yet she's not using any of that to fix the house even tho her reasoning for not having it done before we moved in was that the last tenant wasn't paying rent. But we've paid every single month and still things aren't getting fixed. ​ We also have no central AC, and even with multiple window units our house is still at 77⁰-81⁰ everyday, I have a 4 year old, a 3 month old and 2 dogs. We are absolutely miserable. ​ With all this is in mind we decided to move at the end of this lease. Am I wrong if I don't pay her rent July 1st, even tho we plan to move by then but lease is up July 10th. We plan to contact code enforcement and health department. We were afraid to contact before having a moving plan because of fear that they will tell us we can't stay here and having no where else to go at the time.
